Evaluation of the support and the information activities offered to pregnant women: a national survey.

This study reports the results of an Italian sampling survey carried out in 1999 on the evaluation of the support and the information activities offered to women who delivered, with specific regard to breast-feeding, neonatal care, post-natal period, contraception, and emotional and psychological aspects of care. The survey has been planned and implemented in order to improve the quality of care by the midwives of 23 university maternity hospitals in 12 Italian Regions in collaboration with the Istituto Superiore di Sanità. 1986 women have been interviewed. The response rate was 95%. Among the most critical aspects emerged a frequent utilisation of private antenatal care (75% of the sample referred to a private gynaecologist), a wide geographical variability with frequent penalisation of the women resident in Southern Italy (caesarean section accounts for 28% in the North, for 26% in the Centre and for 46% in the South), a lack of information and knowledge among women who are too often excluded from the decision-making process. Besides, the survey highlighted a lack in continuity of care (that needs a better integration between hospital and field activities) and a progressive medicalization of birth. The authors conclude suggesting the activation of a surveillance system at a regional level that should revise protocols and monitor procedures, outcomes and women's satisfaction degree.
